The Checklist

Please begin by reading the options you have below and begin either Checklist

If your child is 5 years of age or younger please begin with the Speech & Hearing Checklist. At the the end of the Checklist you will be given the option to continue on to the Warning Signs of Stuttering Behavior Form.
       This Speech and Hearing Checklist is to assess the development of your child from 3 months to 5 years of age. After entering your child's name and age you will be shown a set of questions to answer. Following that set of questions you will have the opportunity to answer the next age group's questions if they apply to your child. If you find that your child may be stuttering then you will have the option to continue on to the Warning Signs of stuttering behaviors.
